Abstract

Pelaporan insiden keselamatan pasien (IKP) merupakan hal yang penting dalam program peningkatan keselamatan pasien. Pelaporan insiden dapat membantu menemukan dan menyelesaikan masalah sebagai sebuah pembelajaran (learning). Pelaporan dan analisa insiden keselamatan pasien akan menentukan evaluasi program dan pelayanan kesehatan selanjutnya yang berbasis keselamatan serta mendasari perbaikan sistem pelayanan dan pencegahan terjadinya insiden berulang. Tujuan penelitian untuk mengeksplor implementasi sistem pelaporan dan analisa insiden keselamatan pasien di RSUD dr. H. Andi Abdurrahman Noor Tanah Bumbu. Metode yang digunakan dalam penelitian adalah kualitatif dengan desain fenomenologi. Metode pengumpulan data dilakukan dengan observasi, wawancara mendalam (in-depth interview) dan metode dokumen pada 11 orang kepala ruangan sebagai informan. Analisis data yang digunakan menggunakan teknik analisis Collaizzi’s. Hasil penelitian menunjukan bahwa dalam pelaporan insiden keselamatan pasien belum berjalan optimal, banyak kejadian insiden keselamatan pasien yang tidak terlaporkan yang menjadi hambatan dalam pelaporan insiden karena kurangnya kesadaran petugas kesehatan dalam melaporkan insiden, kurangnya sosialisasi dan pelatihan kepada petugas serta tidak adanya reward yang diberikan kepada petugas kesehatan. Berdasarkan hasil penelitian, diharapkan bagi rumah sakit meningkatkan kembali sosialisasi dan pelatihan kepada petugas kesehatan untuk meningkatkan pengetahuan serta memberikan punishmentterhadap unit yang pelaporannya tidak berjalan dengan baik dan memberikan reward kepada petugas kesehatan agar meningkatkan motivasi untuk melakukan pelaporan insiden keselamatan pasien. Incident reporting is an important element of a patient safety program. It helps to find and resolve problems as a part of learning process. Reporting and analysis of patient safety incidents will determine the evaluation of subsequent programs and health services based on safety and underlie the improvement of the service system and prevention of recurring incidents. This research aims to explore the reporting system and safety incident analysis implementation on patients at Regional Public Hospital of dr. H. Andi Abdurrahman Noor.. This research used qualitative method with phenomenological research design. In collecting the data, in-depth interviews were conducted on eleven informants which was the head of the room. Colaizzi analysis technique was used for data analysis The results showed that the reporting of patient safety incidents has not run optimally, many incidents of patient safety incidents are not reported which become obstacles in reporting incidents due to lack of awareness of health staff in reporting incidents, lack of socialization and training to officers and the absence of rewards given to health staff. Based on the research results, it is expected to improve the re-socialization hospitals and training to health workers to increase knowledgeas well as providing punishment to units whose reporting does not go well and provide rewards to health workers in order to increase motivation for reporting patient safety incident Keywords:
